1- What is premature ejaculation?  

There are numerous definitions of this problem and the one, which appears to be the best one to me, is the following: there is premature ejaculation when a man cannot control his arousal durably and when ejaculation occurs involuntarily. In fact, the problem becomes "critical" if you or your partner is not satisfied with your sexual intercourse. There is no miracle treatment but a properly adapted behavioral therapy can solve premature ejaculation problem.  

2- I would like to know the causes of premature ejaculation.  

There are no medical causes for premature ejaculation apart from pains due to momentary urinary genital and local infections. Therefore, premature ejaculation is essentially of a psycho-behavioral and relational origin.  

3- First of all, I would like to thank you for this service and I would like to ask you some advices and remedies for my problem: my penis extends very slowly and even sometimes not at all, and ejaculation is instantaneous. Thank you for your answer.  

Your problem is the one of premature ejaculation with some difficulties of erection probably due to the fear of failure. If you do not treat your premature ejaculation, you will have more and more difficulties to have a proper erection.  

4- I always ejaculate without being able to control my ejaculation in spite of various techniques, which I tested. I am losing hope to find a solution, what can I do?  

Beforehand, be aware that it is not possible to control his ejaculation but it is possible to control the arousal phase, which precedes ejaculation in order to delay it and to decide “voluntarily” the moment you wish to ejaculate. You must not lose hope because there are new behavioral therapies which will teach you how to control your arousal with 80% to 90% good results.  

5- My wife and I are married since 9 years and I have, during those years, ejaculated 5 times before the penetration because I was very excited. She was caressing my testicles and my penis and, when I felt the orgasm, I did not manage to control it. Do I have a problem and what should I do?  

I can reassure you, because if during those 9 years, you did not manage to control yourself 5 times, you are not suffering from premature ejaculation! Every man has sometimes, during different circumstances, some lost of control and ejaculate quite quickly! If this happens not often, you should not be worry about it. If this would happen more frequently then you should think of a premature ejaculation treatment.

6- My frenulum is short and it pulls on my glans when I penetrate my partner. It is painful and I ejaculate quickly. My doctor told me that a surgical act would solve my problem. I will ejaculate less quickly and I will not have pain anymore. Will this surgical act have other consequences on my sexuality?

Your doctor is right and this surgical act will not have other consequences on your sexuality. However, that is not sure that this surgical act will cure your premature ejaculation problem! 

7- Hello, I am suffering from premature ejaculation and I would like to have some advices. When I am making love to my partner, I do ejaculate quickly. However, I noticed that when I had few drinks, my premature ejaculation seems to disappear. Are there any drugs, which will substitute for alcohol effects?

Ejaculation is a reflex. Alcohol acts on all reflexes so few drinks can delay ejaculation. However, it is evident that alcohol cannot be used as a treatment for premature ejaculation! Some drugs, like antidepressant drugs, have a similar action and sometimes, can be used to delay premature ejaculation. However, these antidepressant drugs have side effects so there are rarely used.

8- During vaginal penetration with my wife, I ejaculate quickly because she likes the backwards and forwards to be quick, so after 2 to 3 minutes I ejaculate, whereas I like it to be slower. What can I do?

Penetrate your wife only when she is already very excited, it will allow her to reach an orgasm even if you ejaculate quickly.

9- I am very embarrassed when I talking about my problem: I ejaculate quickly and after I do not have erection anymore. What can I do to solve my problem? I do not feel like a man because of that. Is surgery can solve my problem?

No, surgery cannot solve premature ejaculation. It is important to know that after ejaculation there is a period where an erection is not possible (refractory period). 10- I am a boy of 18 years old and I had 2 sexual partners in my life : one with who I had sex 3 to 4 times and, since two months, I have a new girl-friend with who I had sex 3 times. With my new girl friend, I always ejaculate quickly whereas I never ejaculate quickly with the first one. Is my problem going to last? I know I did not have many sexual relations but I am afraid of being a premature ejaculator.

Indeed, you did not have many sexual relations so it is a bit early to talk about premature ejaculation. However, if you keep on ejaculation quickly after several other sexual relations, then you will have to treat it quickly because the sooner you decide to do something about it better the treatment will be.
